If a parent/guardian is absolutely unable to accompany the child, then another authorized and responsible adult must accompany them. The parent/guardian must complete and send in this form at the visit. 2019-Permission-to-Accompany-a-Minor
Our schedule allows for several work-in slots every day to accommodate our patient’s needs. If you have a scheduled sick visit for one child and feel another child in your family needs to be seen, please call ahead so we can review the physicians’ schedule to make sure the time is available. We will try our best to accommodate you but must also be cognizant about the families that had already previously scheduled an appointment.
“Joint Custody” means that each parent has equal access to the child’s medical record.
Without a court order, we will not stop either parent from looking at their child’s chart, discuss what each parent told the doctor when they were here last, notify the other parent when a child is being treated, or call the other parent for consent prior to treatment.
Should the issues that come between parents become disruptive to our organization, we will discharge the patient from our care.

Collections Policy
Due to the changing nature of the healthcare market, our collection policies are designed to ensure timely payment for services rendered. We are not a credit agency and cannot therefore carry your debt to our practice. We will send you a statement upon receipt of your insurance payment or explanation of benefits. Any monies due at that time will be expected to be paid upon receipt of your statement.
If payment is not made in a timely manner, our collection process will go into effect and we will no longer be able to schedule well care visits for your child. Upon request for payment with no further response, we will submit your outstanding balance to a collection agency which will prevent us from scheduling further ill visits and functionally discharge your child from our practice. It will also impact on your credit rating.
